Preparing For Your Visit

• Plan to be in the office most of the day!
• If you are a new patient, visit the Patient Portal to complete online medical information.
• Bring all current insurance cards.
• You may be asked to pay any unmet deductible and/or copay the day of service.
• Eat breakfast, bring a snack/lunch as you may be in the office most of the day.
• We have a refrigerator, microwave, and free wireless internet for your convenience.

• Cal Sutter Parking Garage entrance on 29th Street between I and J streets
• Street parking available

Billing

As a courtesy to you, we will bill your insurance carrier if you provide us with complete and correct insurance information. Your insurance policy is a contract between you and your insurance company. We are not a party to that contract. Please bring all current insurance cards to your appointment. Copayments and deductibles are due at the time of service. Once correct payment is received, we will make our contractual adjustments and send you a bill for any outstanding balance. Please notify us immediately if you have any changes to your insurance.

Appointments & Cancellations

With the exception of serious emergencies, it is expected that you keep all of your appointments. If you must change an appointment, please call us as soon as possible to reschedule the appointment. This courtesy allows us to schedule another patient for medical care. Please do not fail to show for an appointment. Failing to show for an appointment or canceling within 24 hours may result in a cancelation fee.

Our office provides on-call service after hours. Call (916) 492-1828 so that the physician on call can be contacted.

If you are experiencing a life-threatening emergency, please call 911 or go to your nearest emergency room.
